I am excited to share the news that I’ve joined Nourish Medical Center, a leading wellness clinic in San Diego, California! With such a wonderful and competent team, I am confident that you will have a seamless continuation of your care, allowing us to pick up right where we left off. I am deeply grateful for your trust and loyalty.
To facilitate this transition, here are a few simple steps to get you all set up!
PREPARE FOR YOUR NEXT VISIT
- Step 1: Visit the Nourish online scheduling portal by clicking here.
- Step 2: Find the “Select” button to the side of my profile and choose 'New Patient, Expanded'.
- Step 3: Browse through the available slots and find a time that best fits your schedule.
- Step 4: Fill in your patient details. Under 'Are you an existing patient?', select 'NO'. When you are done entering your information, click 'Next'.
Under 'Reason for Appointment', please write 'Continuing Care' and specify whether you need a new lab requisition to ensure your lab results are sent directly to me. Feel free to include any additional notes or questions in this section before clicking 'Next'.

FREQUENTLY ASKED QUESTIONS
- Lab Orders: Once your appointment request is made, any necessary follow-up lab orders will be provided. When I place your lab order, you will find your lab requisition in the document section of your Charm Patient Portal. You can print this and take it to your nearest draw station.
- Medication Refills: To request medication refills, simply send a message to Jacqueline Barbato, our Patient Care Coordinator, through your Charm patient portal, detailing the medication name, dosage, and the name of your pharmacy.
- Supplement Refills: If you need a refill on supplements before your next follow-up, visit our Fullscript supplement store here. I have a ‘Dr Raquel’s Favorites’ catalog with some of my frequently prescribed supplements.
- Location: Nourish Medical Center is in San Diego, California. But the good news is, I'm also offering phone and video consultations! This means you can continue care from the comfort of your home or office. For phone consults, expect a call from me at your scheduled appointment time. For video consults, simply click the 'Video Consults' button on the Nourish homepage at your scheduled appointment time.
- Arizona Pellets and In-Person Care: I will still be providing care in Arizona. For in-person appointments or pellet queries, simply call the team at 619-345-3111 or message Jacqueline Barbato in your Charm patient portal to leave a message for me. I'll then contact you to finalize your appointment date and details.
